Membership Application Form

1. I hereby apply for membership in RTO/ERO.
2. I consent to the collection, use and disclosure of any information required to administer my membership in RTO/ERO including personal information such as my Social Insurance Number.
3. (a) For FULL MEMBERS and SURVIVING SPOUSES ONLY. I direct OTPP to deduct membership fees from my pension each January and remit the fees to RTO/ERO. Please note: Depending on the time of year you apply, it may be too late to arrange for your deduction by OTPP. In this case, you will be invoiced a pro-rated amount payable to RTO/ERO for your first year.
(b) FOR ASSOCIATE MEMBERS ONLY. I will remit my annual membership fee directly to RTO/ERO upon receipt of my invoice.
